Brief description of the patient education forms
Patient education as a continuous process that should be documented, evaluated, followed up and built upon. The patient education forms are tools to organize the process of patient education.
The patient health education forms consist of 2 parts. The first part consists of recording patient education pertaining to their diseases, diets, life styles and screenings. The second part is pertaining to education pertaining to medications.
There is an emphasis on teach back technique when counseling patients to assure that they understand the education being imparted.
If for any reason, the patient education is not completed or the health provider feels that there is a need for reinforcement then there is a column to document the need for follow-up. This follow-up will be completed by the same or another provider as soon as possible.
For the education of new medications, it is emphasized that the health providers use the ‘new medication form’ which assures that all the important aspects of a new medication are covered.
There are different ways of imparting patient education as outlined in the key at the end of the form. Different ways can be combined to teach patients in an effective manner.
